Effective June 1, 2015 the Dallas County Clerk's Offices will no longer

accept the Matricula Consular Card as verification of identity for purchase of death certificates or for obtaining confidential records.

 

Obtaining and completing an application for a certified copy of a death certificate:

Individuals requesting or picking up a death certificate must be qualified applicants. Qualified applicants consist of immediate family member by blood or spouse, legal guardian (provide documentation), or legal representative of the family (provide documentation). See Section 181.1(13) of the Texas Administrative Code for what qualifies as an immediate family member.

 

Anyone other than the qualified applicants must have a signed (original) statement written by one of the above qualified applicants authorizing them (naming them) to pick up the death certificate. All third party statements of authorization must be notarized. The statement should be signed by an immediate family member. The statement must give permission to Dallas County Vital Division to release a certified copy of the certificate to the individual name in the notarized statement. The statement must also identify the individual by their full name. The notarized statement must also identify the type of certificate (ex. birth or death) that is to be released. Both parities must submit a valid form of identification. See Section 181.28 of the Texas Administrative Code for a list of acceptable forms of identification.

Application For A Certified Copy Of Death Certificate

The following information is required:

  • Full name as appears on death record
  • Date and Place of death
  • Your relationship to person named on certificate
  • Your mailing address
  • Purpose of record
  • In writing you will need to send a copy of your valid government issued I. D.
  • In person you will need to provide your original valid government issued I. D.
  • A NON-REFUNDABLE SEARCH FEE OF $21 WILL APPLY FOR EACH RECORD NOT FOUND

Cost is $21.00 for the first copy and an additional $4.00 for each copy of the same certificate order at the same time. We do not accept personal checks. Make money order payable to John F. Warren, Dallas County Clerk. Please do not send cash through the mail. Turn around time is 2 to 4 weeks. For more information call 214-653-7099.
