Effective June 1, 2015 the Dallas County Clerk's Offices will no longer

accept the Matricula Consular Card as verification of identity for purchase of birth certificates or for obtaining confidential records.

All third party statements of authorization must be notarized. The statement should be signed by an immediate family member. The statement must give permission to Dallas County Vital Division to release a certified copy of the certificate to the individual name in the notarized statement. The statement must also identify the individual by their full name. The notarized statement must also identify the type of certificate (ex. birth or death) that is to be released. Both parities must submit a valid form of identification.



The following may apply for a Birth Certificate:

Individuals requesting or picking up a birth certificate must be qualified applicants. Qualified applicants consist of immediate family member by blood or spouse, legal guardian (provide documentation), or legal representative of the family (provide documentation). Anyone other than the qualified applicants must have a signed (original) statement written by one of the above qualified applicants authorizing them (naming them) to pick up the birth certificate. This statement must include a copy of the qualified applicant's government issued ID. The person authorized to pick up the birth certificate will also need to present their valid government issued ID.

Please mark on your request application if you are wanting the Long Form Birth Certificate or the Short (Abstract) Form Birth Certificate.

Long Form Birth Certificate:

The long form birth certificate is the original birth certificate which contains detailed information.

  • person's name, sex, date of birth, and place of birth.
  • parent(s) name, date of birth and place of birth.
  • name of the hospital, doctor and time of birth.
  • State File Number and the Local File Number that the record has been registered with.

The long form birth certificate is usually needed for newborns, passport applications and Indian Registry.

Long form birth certificates and death certificates are available for the following cities, (Addison, Balch Springs, Carrollton, Cedar Hill, Cockrell Hill, Coppell, DeSoto. Duncanville, Farmers Branch, Garland, Glenn Heights, Grand Prairie, Highland Park, Hutchins, Irving, Lancaster, Las Colinas, Mesquite, Richardson, Rowlett, Sachse, Seagoville, Sunnyvale, University Park, and Wilmer).

Short (Abstract) Form Birth Certificate:

The short (abstract) form birth certificate contains basic information.

  • person's name, sex, date of birth, and county of birth.
  • parent's name(s), the State File Number and the filing date.

     The short (abstract) form can be used to apply for a government issued ID, enrollment in school or sports, and for work related issues. When applying, please indicate the type of form you are requesting as no refunds will be issued.

Application For a Certified Copy of Birth Certificate

The following information is required:

  • Full name as it appears on the birth record
  • Date and county of birth
  • Parent(s) name, including mother's maiden name
  • Your relationship to the person named on the certificate
  • Your mailing address
  • Purpose of record
  • Full name of hospital where the birth occurred
  • In person, you will need to provide your original valid government issued ID
  • In writing, you will need to send a copy of your valid government issued ID
  • Cost per copy is $23.00 money order, payable to John F. Warren, Dallas County Clerk’s Office

  • Turn around time is 2 to 4 weeks when applying by mail please send money order only. If applying in person, you may pay in cash and receive the record while you wait.
  • Credit cards will not be accepted as a form of payment by mail.
