COURT ORDER:  Approving refunds to taxpayers, as listed, for the period ending March 15, 2011, in amounts over $5,000.00 for erroneous or excessive tax payments.

COURT ORDER:  Approving the Texas Department of State Health Services (DSHS) Contract No. 2010-035150-001A, CPS-Laboratory Response Network-HPP, for the term of July 1, 2010 through June 30, 2011, and authorizing the County Judge to sign the contract and all related documents on behalf of Dallas County.

COURT ORDER:  Approving the additional award from Atmos Energy Corporation, in the amount of $38,504.63, under the Share the Warmth Program, for assistance with energy costs for financially distressed Atmos Energy account holders.

COURT ORDER:  Approving the award of reallocated FY 2010 Ryan White Part A Formula grant funds in the amount of $260,610, to service providers as outlined in Attachment A, and authorizing the County Judge to sign the contract agreements with the service providers on behalf of Dallas County.

COURT ORDER:  Approving the FY 2011 Ryan White Part A-Minority AIDS Initiative (MAI) grant Administrative Agency and Quality Management budgets in the amounts of $458,039, as outlined in Attachment A.

COURT ORDER:  Approving the FY 2011 Ryan White Part A Formula Administrative Agency and Quality Management budgets in the amounts of $4,972,173, as outlined in Attachment A.

COURT ORDER:  Approving the 2011-2012 LIHEAP Weatherization Assistance Program Contract No. 81110001163, with the Texas Department of Housing and Community Affairs, in the amount of $2,147,860, for a term effective April 1, 2011 through March 31, 2012, and authorizing the County Judge to electronically sign the Contract and all related documents on behalf of Dallas County.

COURT ORDER:  Approving the outlined Interlocal Agreement (ILA) for Forensic Laboratory Services between Dallas County and the City of Dallas, and authorizing the County Judge to sign the agreement on behalf of Dallas County.

COURT ORDER:  Ratifying the submission of the applications to the Criminal Justice Division (CJD) for the Violent Crimes Against Women and the Juvenile Justice grant funding in the amount of $80,000 per grant; granting authorization to accept if awarded said funding, and authorizing the County Judge to sign, accept, reject, alter, or terminate said grants on behalf of Dallas County and that the County Auditor be designated as the fiduciary agent for said grants.

COURT ORDER:  Authorizing the final twelve (12) month extension of (Bid No. 2008-064-3473 – Annual Contract for the Purchase of Rechargeable Batteries) as awarded to Battery Zone, Inc., for the period of May 14, 2011 through May 13, 2012, and authorizing all County documents/payments to reflect accordingly.

COURT ORDER:  Authorizing the final twelve (12) month extension of (Bid No. 2009-049-4256 – Annual Contract for the Rental of Vehicles for Group A) (Passenger Vehicles, Minivans, SUVs) only as awarded to Enterprise Leasing Company of DFW, effective March 17, 2011 through March 16, 2012, and authorizing all County documents/payments to reflect accordingly.

COURT ORDER:  Authorizing the first twelve (12) month extension of (Bid No. 2010-040-4982 – Annual Contract for Photographic Supplies) as awarded to Southpoint Photo Imaging Supplies, Inc., for the period of June 1, 2011 through May 31, 2012, and authorizing all County documents/payments to reflect accordingly.

COURT ORDER:  Authorizing the first twelve (12) month extension of (Bid No. 2010-043-5005 – Annual Contract for Fire Extinguisher Inspection and Service) as awarded to Simplex Grinnell for the period of May 4, 2011 through May 3, 2012, and authorizing all County documents/payments to reflect accordingly.

COURT ORDER:  Authorizing the first extension of (Bid No. 2010-049-5033 – Annual Contract for the Purchase of Clearview Rapid HIV ½ Stat Pak Kits) as awarded to Alere North America for the period of May 12, 2011 through May 11, 2012, and authorizing all County documents/payments to reflect accordingly.

COURT ORDER:  Authorizing the livestock named here (3 donkeys, 1 horse) as representative of the items to be sold in the upcoming online auction, and authorizing all County departments/inventories to reflect accordingly.

COURT ORDER:  Authorizing the Purchasing Department to solicit Request for Proposals for Merchant Services for Electronic Payments and Bank Card Processing in accordance with Texas Local Government Code 262.030.

COURT ORDER:  Authorizing the cancellation of (Bid No. 2011-039-5448 – Annual Contract for an Enterprise Wide Preventive Maintenance Agreement for Scanners), rejecting all bids received from award consideration, and authorizing staff to revise the specifications and re-solicit bids at a later date.

COURT ORDER:  Approving the outlined Request for Statements of Qualifications for use in obtaining Consultant Engineering Services, and authorizing the Purchasing Agent in collaboration with Public Works to begin the process of selecting the highest qualified consultant engineering firms.

COURT ORDER:  Authorizing: (1) the Public Works Department of Dallas County, Texas to offer Property Owner Elvin L. Hayes, Jr., and any and all owners and claimants of the property rights and interests in and to a certain parcel of land as described and identified in Exhibit “A”, to purchase 1,552 square feet of Permanent Easement, improvements and cost to cure at the compensation amount of $7,569.00; (2) a settlement increase in the amount of $1,242.00 be paid to compensate Property Owner for additional fence request to meet City of DeSoto fence standard for this project, to be reimbursed 100% by the City of DeSoto; (3) authorizing the County Judge and Commissioners to execute the outlined Contract Agreement and any other documents to consummate this transaction and; (4) authorizing the County Treasurer to remit payment in the not to exceed amount of $8,811.00, to be paid from fund 0196, project 08201.

COURT ORDER:  Authorizing and directing the County Judge to execute the outlined Work Order Number four (4) to the existing contract between Dallas County and Freese & Nichols, Inc., in an amount not to exceed $234,400.00, to be paid from Fund 196, Project 8201.

COURT ORDER:  Authorizing and directing the County Judge to execute the outlined seven (7) separate contracts for indefinite delivery and quantity of laboratory services (one (1) year) between Dallas County and Globe Engineers, Inc., Alliance Geotechnical Group, Inc., Professional Services Industries, Inc., Alpha Testing, Inc., TEAM Consultants, Inc., Mas-Tek Engineering and Associates, Inc., and Rone Engineering Services.

COURT ORDER:  Authorizing the outlined Addendum No. 3 for the professional services agreement with SFN Professional Services LLD d/b/a Tatum and authorizing the County Judge to execute the documents on behalf of Dallas County.
