The Human Resources/Civil Service Department is pleased to provide this resource to assist you, our employees, with your planning and preparation for Open Enrollment for Plan Year 2016. On this web page, you will find information on: (1) what you need to do prior to enrolling and (2) any 2016 plan changes. We encourage you to spend time closely reviewing the general information and plan changes before you enroll. By preparing for your enrollment, your experience will be fast and easy!
Open Enrollment gives you the annual opportunity to
make changes to your benefit elections.
WHEN: November 1st - November 16th, 2015
HOW: Online via ESS (Employee Self Service)
Beginning on November 1, 2015, you will be able to make changes on-line by utilizing ESS. Changes to medical, dental, vision, flexible spending accounts, long term care, etc. After November 16th, you’ll need to contact Human Resources to make corrections until November 20th.
Any changes you make during Open Enrollment
will take effect January 1, 2016.

IMPORTANT INFORMATION FOR THE UPCOMING PLAN YEAR:
Surcharge Affidavit: To be completed if you are enrolling your spouse in your employer medical plan (PPO Plan or HDP, including retiree PMD/PSD). If you are NOT enrolling your spouse in your employer medical plan this form is not needed. If you fail to complete this form or are late turning it in and you enroll your spouse in your employer medical plan, a $200.00 per month surcharge will be assessed.
