Before visiting one of our three locations, please ensure you have the following supporting documentation and materials:

• Certified Birth Certificate with State, City, or County Seal or previous U.S. Passport, or Certificate of Naturalization, or Certificate of Citizenship, or Consular Report of Birth Abroad. Hospital birth certificates are NOT Acceptable.
• Two identical, recently made 2" x 2" photos with image size between 1 and 1-3/8 inches from bottom of chin to top of hair with plain white background. Photos must be clear, front view, full face, taken in normal street attire (no hats or dark glasses). Passport photos are available at all three Dallas County Passport locations for $10.00 cash.
• Valid drivers license, military ID, or government ID (Federal, State, or Municipal).
• All applicants, regardless of age, must appear in person. Applicants 16 and 17 years of age must apply in person and be accompanied by one of the parents named on the birth certificate. Minors 15 and younger must apply in person and be accompanied by BOTH parents named on the birth certificate. PLEASE NOTE: In the case of passport renewals for minors 15 and younger, both the expired passport and a certified birth certificate must be presented.
  
• Certified proof of name change (required only when applicable).
• A fee for applicants 16 years of age and older in the amount of $75.00 by check or money order ONLY payable to Department of State. The fee for minors 15 years of age and younger is $60.00 check or money order ONLY.
• $25.00 (cash ONLY) for execution fee.
• Your Social Security number.
• ALL REQUIREMENTS ARE MANDATED BY THE U.S. DEPARTMENT OF STATE.

Please keep your receipt until you have received your passport and submitted documents.
