notice. Call to schedule an appointment. Current picture ID for each household
member 18 years of age or older. Current benefits award letter, if applicable TANF,
SSI, etc. Current lease or mortgage agreement. Original late notice showing
delinquent amount now due for rent/mortgage or utilities.
